Dr Sicknote does not guarantee that an eConsultation is the appropriate course of treatment for your particular health care problem. You agree to contact your own doctor immediately should your condition change or your symptoms worsen. If you require urgent care, you should contact your local emergency services immediately.
The service offered is done so on an "as is" basis and used by the patient solely at his or her own risk. Dr Sicknote may, among other things withdraw, suspend or discontinue any functionality or feature of its services.
All of the Treating Providers who deliver Services through Dr Sicknote are independent professionals solely responsible for the services each provides to you.
Dr Sicknote does not practice medicine or any other licensed profession, and does not interfere with the practice of medicine or any other licensed profession by Treating Providers, each of whom is responsible for his or her services and compliance with the requirements applicable to his or her profession and license.
Neither Dr Sicknote nor any third parties who promote the Services or provide you with a link to the Service shall be liable for any professional advice you obtain from a Treating Provider via the Services.
When you register to obtain a product or service on this site, you accept the terms. Your use of any such product or service offered on the site is governed by these terms. Except as provided in the terms, Dr Sicknote does not warrant that any product descriptions or content contained within the site is accurate, current, reliable, complete, or error-free.
As part of your registration you are required to provide us with certain personally identifiable information. It is your responsibility to update this information so our records are current, complete and accurate.
Dr Sicknote reserves the right to cancel your registration without notice and at it's sole discretion.
Modification of Medical Certificates or Any other Document issued to you by a Practice or Doctor serving you as part of this Service is Strictly Prohibited
By using this service you agree not to amend, modify, copy, transfer or share any and all documents issued to you as part of this Service with the intention of altering the documents issued to you for your use or by other persons. This includes Medical Certificates, Health Certificates or Statements, Carer or Attendance Certificates, Medical or specialist referrals or any other documents covering the routine medical procedures offered as part of this Service. Any attempt to modify a document will be construed as an intention to commit a fraud and will render the document null and void. In such circumstances, we reserve the right to commence legal action against you. Such actions will also constitute grounds for immediate cancellation of registration and ongoing service and site usage.
Documents issued to you by the practicing doctor or practice from this Service can only be copied or reproduced as to satisfy the person or organisation requesting its contents or the verification of its contents or in fulfilling the request of its intended use.
Dr Sicknote stores data online and as part of the terms you agree to your information being stored online in a secure location and accessed in this way. To facilitate your effective treatment we will supply your treating doctor with the information we have on record about you and provided to us by you.Booking Fee Refund Policy
Dr Sicknote charges patients a fee for its services. This fee is charged at the time of eConsultation and will be refunded if the doctor is unable to issue a medical certificate for any reason.
Medical Certificates will also not be issued if the doctor believes there is a deliberate intent by the patient to commit a fraud in seeking the Medical Certificate. However, the doctor will issue a statement to the patient setting out relevant recommendations for either improving health or a request that the patient seek further medical treatment. This may include seeking a second opinion via a face-to-face appointment with a local doctor.
In some circumstances for example, a physical examination, blood tests, or diagnostic imaging may be required. These elements cannot be delivered via an e-health appointment. If the doctor determines that any or all of these are required, and in the patient’s best interest, the doctor will direct you to make an appointment for an in practice consultation with your local general practitioner.
As part of this service, doctors will not issue medical prescriptions to patients. If you require medical scripts, we recommend that patients visit their local GP. Where a patient is currently being treated by a local GP with prescription medicines, we would recommend that a patient see their local GP or treating doctor for the issue of further prescription based medications.
The site and the content within the site are the property of Dr Sicknote or its suppliers and are protected by laws relating to intellectual property, including copyright laws, and international treaty provisions. The compilation, organization and display of the content as well as all software and inventions used on and in connection with the site are the exclusive property of Dr Sicknote. Dr Sicknote reserves all rights in the site and its content not specifically granted in any agreements with Dr Sicknote or in the terms.Informed Consent
E-health is the delivery of health care services using electronic processes and communication, where the patient and the health care professional are not in the same physical location.
During your eConsultation with a Treating Provider, details of your medical or health history and personal health information may be supplied by you through the use of text, video or audio technology.
Depending on your medical or health history and/or specific complaint, you may be asked to provide information through biometric devices such as digital thermometers or blood pressure cuffs. You may also be asked for proof of identity with a driver’s license or other legal document. We reserve the right to deny Service provision where the doctor believes proof of identity has not been satisfactorily established.
The E-health Services you receive from Treating Providers are not intended to replace a primary care physician relationship or be your permanent or primary care medical relationship. You should seek emergency help or follow-up care when recommended by a Treating Provider or when otherwise needed, and continue to consult with your primary care physician and other healthcare professionals as recommended.
Although the electronic systems we use will incorporate network and software security protocols to protect the privacy and security of health information, in rare instances, security protocols could fail, causing a breach of privacy of personal health.
In consideration of Dr Sicknote (which expression shall be taken to include Dr Sicknote’s servants, agents, officers and directors, successors or assignees) agreeing to permit me use of the site and it’s ancillary services:

Dr Sicknote cannot ensure the security or confidentiality of messages sent by email.

Dr Sicknote reserves the right to take any and all action, as it deems necessary or reasonable, regarding the security of the Site and your Account Information. You may not use anyone else’s account at any time. We also reserve the right to request proof of identity at the commencement of each appointment. If we are not satisfied by this proof we may deny offering services to you through the Site.
The Site and its entire contents, features and functionality (including but not limited to all information, software, text, displays, images, video and audio, and the design, selection and arrangement thereof), are owned by Dr Sicknote, its licensors or other providers of such material and are protected by Australian and international copyright, trademark, patent, trade secret and other intellectual property or proprietary rights laws.
Certain of the names, logos, and other materials displayed on the Site or in the Services may constitute trademarks, trade names, service marks or logos (“Marks“) of Dr Sicknote or other entities. You are not authorized to use any such Marks without the express written permission of Dr Sicknote. You acknowledge and agree that the Treating Providers using the Site are solely responsible for and will have complete authority, responsibility, supervision, and control over the provision of all medical services, advice, instructions, treatment decisions, and other professional health care services performed, and that all professional health care services will be provided and performed exclusively by or under the supervision of Treating Providers, in their sole discretion, as they deem appropriate.
